The Case Manager is responsible for:

• Assessing the areas of the consumer’s life where assistance is needed.
• Offering and coordinating resources, and services for the person.
• Monitoring service delivery and working with providers to ensure the consumer’s needs are met.
• Provide person centered service planning with person and creating Coordinated Consumer Support Plans.
• Attend regular meetings with the person and their interdisciplinary teams.
• Travel to different sites in different counties for meetings.
• Case managers will be providing case management for people on EW, AC, RULE 185, CADI, CAC, and IDD waivers.
• Other duties as assigned.

Case Manager Requirements:

· Applicants must have a valid driver’s license and acceptable driving record as determined by Bestview Care Options policy. Applicants must have a vehicle that is covered with current insurance. Applicant must have a reliable vehicle.

· Applicants must successfully clear local and/or stated background checks.

· ‘Preferred Qualifications: One to two years’ experience in service coordination or case management is preferred, but not required.

· Applicants will be required to have internet at home.

Job Type: Full-time

Salary: $49,000.00 - $65,000.00 per year

Schedule:

• 10 hour shift
• 8 hour shift

COVID-19 considerations:

All case managers carry masks and PPE items.

Ability to commute/relocate:

• Roseville, MN 55113: Reliably commute or planning to relocate before starting work (Preferred)

Education:

• Bachelor's (Required)

Work Location: In person/ Hybrid model
